Knee pain can change how you walk, work, exercise, and enjoy life. It may keep you from hiking, running, cycling, skiing, lifting weights, playing sports, or simply moving through the day with confidence.
At Sierra Spine & Fitness, we do not begin by guessing which treatment you need. We begin by identifying the tissues involved, evaluating how your knee functions, and determining why it is not recovering.
Our approach combines a detailed clinical examination, diagnostic musculoskeletal ultrasound when appropriate, biomechanical assessment, chiropractic and hands-on care, non-surgical regenerative therapies, and progressive rehabilitation.
The goal is not simply to reduce your pain. It is to restore function, rebuild capacity, and help you return to the activities that matter to you.
What Is Causing Your Knee Pain?
The knee is influenced by cartilage, ligaments, tendons, muscles, joint surfaces, and the mechanics of the hip, ankle, and foot. Different problems can cause similar symptoms, which makes an accurate diagnosis essential.
Common conditions we evaluate include:
- Meniscus injuries and degenerative meniscus tears
- Anterior cruciate ligament, or ACL, sprains
- Medial collateral ligament, or MCL, sprains
- Lateral collateral ligament, or LCL, sprains
- Knee osteoarthritis
- Patellar and quadriceps tendon disorders
- Joint irritation and swelling
- Patellofemoral pain
- Bursitis
- Movement- and load-related knee pain
Meniscus Tears
The menisci are two cartilage structures located between the thigh bone and shin bone. They help distribute load, absorb force, improve joint stability, and protect the articular cartilage.
Meniscus injuries may occur suddenly during twisting, pivoting, or deep bending. They can also develop gradually as the tissue becomes less resilient over time.
Symptoms may include:
- Pain along the inside or outside joint line
- Pain with twisting, squatting, or kneeling
- Swelling or stiffness
- Clicking, catching, or locking
- Difficulty fully bending or straightening the knee
- A sense that the knee may give way
- Pain during or after walking, hiking, or running
Not every meniscus tear requires surgery. Treatment depends on whether the tear is traumatic or degenerative, its location and pattern, the presence of true mechanical locking, associated ligament injury, and the effect it has on function.
ACL Sprains and Tears
The anterior cruciate ligament helps control forward movement and rotation of the shin relative to the thigh. ACL injuries often occur during cutting, pivoting, sudden deceleration, awkward landings, or direct trauma.
Signs of a significant ACL injury may include:
- A popping sensation at the time of injury
- Rapid swelling
- Loss of motion
- Instability or giving way
- Difficulty pivoting or changing direction
- Reduced confidence in the knee
Some ACL injuries require surgical consultation, particularly in patients with substantial instability or demanding athletic goals. Other cases may be managed non-surgically with carefully progressed rehabilitation.
Our role is to identify the severity of the injury, recognize when advanced imaging or orthopedic consultation is needed, and help restore motion, strength, coordination, and stability.
MCL and LCL Sprains
The medial collateral ligament supports the inside of the knee, while the lateral collateral ligament supports the outside. These ligaments help resist side-to-side stress.
An MCL injury commonly results from a force that pushes the knee inward. An LCL injury may occur when force pushes the knee outward or as part of a more complex knee injury.
Symptoms may include:
- Pain along the inside or outside of the knee
- Localized swelling and tenderness
- Pain with side-to-side stress
- Instability in more significant injuries
- Difficulty walking, cutting, or changing direction
Musculoskeletal ultrasound can be particularly useful for examining the accessible portions of the MCL and LCL, assessing fiber continuity, and identifying surrounding swelling. More complex injuries may require MRI or orthopedic evaluation.
Knee Osteoarthritis
Knee osteoarthritis involves changes in the articular cartilage, joint surfaces, synovium, and surrounding tissues. It does not mean that the knee is simply “worn out” or that pain and disability must continually worsen.
Symptoms may include:
- Knee pain with walking or standing
- Morning stiffness
- Difficulty with stairs
- Reduced motion
- Swelling
- Grinding or crepitus
- Difficulty rising from a chair
- Reduced tolerance for exercise or recreation
The severity of changes on an X-ray does not always match the severity of a person’s symptoms. Strength, joint mobility, inflammation, activity level, sleep, body composition, and confidence in movement can all influence how the knee feels and functions.
Our objective is to improve the health and capacity of the entire knee—not merely treat an image.
We Identify What Is Actually Involved Before Deciding How to Treat It
Knee pain is not a diagnosis. Pain in the same location can arise from a meniscus, ligament, tendon, bursa, joint surface, muscle, or referred source.
At Sierra Spine & Fitness, your evaluation may include:
- Orthopedic and ligament testing
- Meniscus provocation tests
- Strength and range-of-motion assessment
- Evaluation of swelling and joint irritability
- Squatting, stepping, walking, and balance analysis
- Assessment of hip, ankle, and foot mechanics
- Diagnostic musculoskeletal ultrasound
- Digital X-rays when appropriate
- Referral for MRI or orthopedic evaluation when necessary
Musculoskeletal ultrasound allows us to visualize many knee structures in real time. It can be useful for assessing the collateral ligaments, quadriceps and patellar tendons, bursae, joint fluid, superficial meniscus margins, and other accessible tissues.
Ultrasound does not replace MRI for every suspected internal knee injury. When the ACL, deeper meniscus structures, articular cartilage, or other internal tissues require more complete evaluation, MRI may be recommended.
Diagnostic certainty also means recognizing the limits of each test and choosing the right imaging for the right question.
Non-Surgical Technologies, Selected for Your Diagnosis
Sierra Spine & Fitness offers several non-surgical technologies that may be incorporated into care when appropriate. Treatment is selected according to the diagnosis, tissues involved, symptom irritability, and your functional goals.
Focused shockwave delivers acoustic energy to a precisely controlled area and depth. Depending on the diagnosis, it may be used to target selected knee structures and support biological activity, pain modulation, circulation, and tissue remodeling. Research has also reported improvements in pain and function among patients with knee osteoarthritis, although results vary and treatment must be individualized. Focused shockwave is different from radial pressure-wave therapy and can deliver therapeutic energy to deeper, more localized tissues.
Radial shockwave distributes mechanical energy across a broader and more superficial area. It may be used for surrounding muscles, tendons, and other tissues that influence knee function. Focused and radial shockwave may be used separately or together, depending on the treatment objective.
Extracorporeal Magnetotransduction Therapy uses rapidly changing electromagnetic fields to interact with tissues at the cellular level. EMTT is non-invasive and may be combined with focused shockwave and rehabilitation for selected knee conditions. It does not require needles or direct mechanical pressure over the painful structure.
Therapeutic red and infrared light energy may be used to support cellular activity, circulation, and the modulation of pain and inflammation. Laser therapy is comfortable and non-invasive. It may be incorporated into a broader plan for knee osteoarthritis, tendon-related pain, ligament sprains, and other appropriately selected conditions.
The Knee Does Not Function in Isolation
The knee does not function in isolation. Hip strength, ankle mobility, foot control, balance, and the movement of the pelvis can all affect how forces travel through the knee.
Chiropractic and hands-on care may help:
- Improve mobility in the knee, hip, ankle, or foot
- Address restrictions that interfere with normal movement
- Reduce compensatory stress
- Improve comfort during rehabilitation
- Restore more efficient lower-extremity mechanics
We also evaluate how you squat, step, walk, balance, and control the leg. For active patients, we may examine running, jumping, landing, and change-of-direction mechanics.
Hands-on care can create better movement options. Progressive rehabilitation teaches your body how to use them.
Technology Can Stimulate Tissue. Loading Teaches It What to Become.
Shockwave, EMTT, laser, chiropractic, and manual therapy may help reduce barriers to recovery. But lasting improvement also requires the knee to become stronger and more capable.
Your rehabilitation program may include:
- Early range-of-motion exercises
- Isometric strengthening
- Quadriceps and hamstring loading
- Hip and calf strengthening
- Balance and proprioceptive training
- Squats, step-ups, and split squats
- Progressive single-leg exercises
- Walking, hiking, or running progression
- Jumping, landing, and change-of-direction training
- Work- or sport-specific preparation
Exercises are selected according to your diagnosis and progressively adjusted as your knee adapts.
The objective is not to protect the knee forever. It is to build a knee that can confidently handle more.

Our approach follows four connected steps:
Find the Problem
Identify the involved tissues and the mechanical, functional, and loading factors contributing to your knee pain.
Create the Conditions for Recovery
Use the appropriate combination of chiropractic care, hands-on treatment, shockwave, EMTT, laser, activity modification, and other strategies.
Give the Knee the Right Stimulus
Progressively restore mobility, strength, balance, coordination, and tolerance to load.
Build the Capacity to Do More
Prepare your knee for the demands that matter to you—from comfortable daily movement to hiking, running, lifting, working, skiing, and sport.
See it. Treat it. Load it. Restore it.
Do I Need Surgery?
Many cases of knee osteoarthritis, collateral ligament sprains, and degenerative meniscus-related pain can be managed without surgery. Some ACL tears and traumatic meniscus injuries may also be appropriate for structured non-surgical rehabilitation.
However, surgery or orthopedic consultation may be necessary when there is:
- A displaced or repairable traumatic meniscus tear
- Persistent true mechanical locking
- Significant instability
- A complete ligament rupture
- Multiple injured knee structures
- A fracture
- Progressive neurological or vascular symptoms
- Failure to improve with appropriate conservative care
If we believe you need advanced imaging, an orthopedic opinion, or another form of care, we will tell you.
Pain Relief Is Not the Finish Line
Pain relief matters. But it is not the finish line.
The real goal may be walking farther, climbing stairs, working without restriction, returning to the gym, hiking the trail, running again, skiing, cycling, or playing your sport with confidence.
At Sierra Spine & Fitness, we combine advanced diagnosis, non-surgical regenerative therapies, chiropractic care, and progressive rehabilitation to help you reduce pain, restore function, and build the capacity to do more.
